ROOM ADDITIONS

While at the court, finding out what has become of a document I requested two weeks ago, I find what I have noticed at some other offices, here in Hawaii. That is, official documents that do not contain space for pertinent information, ie; dates, times, file, or document numbers, etc.; and procedures that make loss and destruction of documents an acceptable hazard of clerical professionalism.

DESIRED EFFECT




The form used to request the document has no space for the date. The date is not requested. When I submit the form, there is no type of receipt given.

I believe, this is to give elbow room for excuses, forgery and fraud, by the deletion of information; the addition of information; and loss, toss, or out right destruction.
